Actions taken by master when any incident happens onboard

• 1. Take immediate action to control damages
  • Initial report
    • Phone call to the office( [phone]) or DPA([phone])
    • If there is still no answer, Master should call company GM Capt. Fang via mobile: [phone]
    • After the incident is initially controlled, the Master shall fill in “SQR-CAP007 Initial Report and submit to S.Q.A department and copy to DPA
  • Refer to 1.2 of EMG 02 of HSQE-06 to take immediate action
  • Refer to HSQE-06 for different emergencies.
• 2. Check damages/assess the ship conditions
  • Check the damages on your own ship
  • Basis on the present damages and situation and its trend of development, the master must assess the ship's conditions (refer to 1.3 of EMG 02 of HSQE-06)
• 3. Collect evidence to make SOF and Protest
  • Collect evidence
    • Copy related documentation
      • Deck/engine logbook
      • Related ship's certificates
      • Related operation records
    • Save VDR and CCTV if necessary
    • Photos of damaged equipment
  • Make master's SOF and Protest
• 4. Investigation
  • When situation is fully under control, master shall organize the investigation to find out the cause of the accident
• 5. Follow-up report
  • Based on the investigation, the Master shall fill in SQR-CAPT008 Accident/incident followed-up report”and submit to S.Q.A department and copy to Designated Person Ashore.
